Which document have you lost?
The two are often confused. The Registration Book is formally the "Vehicle Registration Document" and records the vehicle and its owner. The Road Licence is formally the "Vehicle Licence" and is the proof that the vehicle may be used on the road. A duplicate of either is applied for using the same form: TD 151.

Lost your Vehicle Registration Document
If a registered owner loses the Vehicle Registration Document, they must apply in person or through an agent authorized by a letter of authorization to any Transport Department licensing office, submitting a completed "Application for a Duplicate Vehicle Registration Document / Vehicle Licence" (form TD 151) together with the documents specified on the form.
If the registered owner is a company, the agent must produce the company's letter of authorization and the Certificate of Incorporation.
Lost your Vehicle Licence
If the Vehicle Licence has been lost, you must bring a completed form TD 151 and the documents specified on the form in order to apply for a duplicate Vehicle Licence.
For company owners, the agent must produce the company's Certificate of Incorporation; a copy verified by a registered accountant or lawyer is also accepted.
What it costs
Under the Transport Department fee schedule, a duplicate Vehicle Registration Document and a duplicate Vehicle Licence each cost $60.
If you are also cancelling the vehicle registration, that procedure itself carries no handling fee; the duplicate is a separate charge.
Can I arrange scrapping while waiting for the duplicate?
We could not find an explicit provision in the official documents about arranging scrapping while a duplicate is being processed, so we will not speculate.
In practice, cancelling a vehicle registration requires the originals of the Registration Book and the Vehicle Licence, so the safest route is to get the documents replaced first.
